One of our certified nurses will visit you for a complimentary consultation and evaluation with the aim to create a care plan, whether you’re at home or in a hospital room. Receive a Tailored Care Plan:

Our home care agency will craft a care plan tailored to your specific needs and budget. Recognizing that needs can evolve, we’ll frequently reassess and modify your care plan. Every care plan is supervised by a certified nurse.

We provide support to clients under NDIS, My Aged Care, DVA, and NIISQ. All support staff are deemed competent by our Registered Nurses ensuring all participants are provided with high-quality care. Our staff are trained in managing high intensity clients and complex behaviours. Our dedicated nursing team are experienced in continence management and complex wound care. We aim to help clients receive the care they need in the dignity of their own homes.

2. Do you provide in-home aged care services?

Resolute Care is registered under Support-At-Home and provides in-home care to participants registered under the SAH program. Our services include personal care, domestic assistance, transport, community and social participation, nursing support, and continence management. Our services are tailored to each person’s needs and goals.

3. Which areas do you service?

Resolute Care Group’s head office is based in Townsville, although we provide supports in Ayr, Brisbane, Gold Coast, and other surrounding areas.

Our Associated Provider and Support Coordination networks allows our team to connect with organisations in the wider community.

4. How do I access My Aged Care services?

To access My Aged Care services, you may need an assessment through My Aged Care. Our team can help you understand the process and discuss available support options.
